Two - Part Snap - Out Request ( Form Rcs )

A shrink-wrapped package of 100 two-part, no carbon required form (5 1/2" x 4"). These are used when a person requests to join a church by "letter". The church clerk sends a Request for Letter postcard to the petitioner's former church. The form is perforated so that the top "snaps" off leaving a paper form for the church clerk/filing and a 5 1/2" x 3 1/2" postcard-size form for mailing. No postage markings are on the postcard.
